Return books borrowed from the faculty library and after manage to complete the alumni check out list. Fill in your address, have library confirms that you have returned books, undergo physical medical check up, no test are required. Have doctor confirm your medical check up into alumni check out list. Choice of the doctor is up to you e.g. http://www.ordinace-karlin.cz/pages/english/home-page.php Sign alumni check out list at the bottom of paper and deliver it to the study department before you are leaving home (without the ceremony) or before the graduation ceremony. Return your ISIC card to the study dept. too. Please remember that the date of the signature of library and of the doctor must be after your last state exam!
